APOST Quality Capacity Mini-Grants
The purpose of APOST Quality Capacity Mini-Grants is to support Quality Campaign Members in their continuous quality improvement efforts. Funding requests must directly reflect the items from your agency’s Quality Self-Assessment (QSA) Action Plan that seek to improve the program’s:
1) Structure and management
2) Positive connection
3) Safety and health
4) Activities
Mini Grants will be accessible via the Mini Grant Online Application Form twice a year at a maximum of $3,000.00 per grant. Note: mini grants will no longer be accepted on a rolling basis. Additionally mini grants will be reviewed by a volunteer review team. This will not be a group of QC members.
Mini-Grant Timelines
Spring/Summer 2019
Applications open: Thursday, March 14th
Applications due: Friday, April 19th
Notice to approve/deny: Friday May 24th
Fall 2019
Applications open: Thursday September 12th
Applications due: Friday, October 18th
Notice to approve/deny: Friday November 15th
Action Plan Packets
Members are required annually to complete an action plan update and to create a new action plan for the coming year. Please see below for the action plan packet that corresponds with the year of your current term of membership. You will need a copy of your previous action plan to complete the update portion. Additionally, when creating your newest action plan, please remember that you only need to set goals for 3-5 indicators. When finished, you can email it to apostpgh@gmail.com. Please note: these are Microsoft Word documents and must be downloaded from a computer. They are not compatible with smart phones.

Renewal Information
The type of renewal form you complete depends on what has happened in your organization since you first applied. Please read the following questions to determine which renewal type is right for you.
Since completing your very first Quality Self-Assessment (QSA), have you had:
- Any major changes in staffing?
- Any major changes in programming?
- Any major structural changes, such as mergers with other organizations?
If you answered yes to any of the questions above, then you must complete another Quality Self-Assessment, which is Renewal Type A. If you have not had any changes since completing your very first QSA, then you will complete Renewal Type B. Please note that these are Microsoft Word documents and must be downloaded from a computer. They are not compatible with smart phones.
